Payments are processed on the 20th of each month and will continue until the balance is paid in full.

Cost to Participate
• $25 enrollment fee per semester (ACH & credit card)
• $25 returned payment fee for each payment returned

Tips for Completing the Agreement
• At the top, select the correct semester or term
• Attach a voided check if applicable
• The Responsible Party name listed in section 2 must
match the signature and name listed on the voided check
